Person-Centered Screening Conversation Map

Collaborative Screening

TOOL

We have many reasons to ask people about their individual and family circumstances during health screenings. Yet for every reason to ask, patients have just as many reasons to hesitate to share personal information.

Collaborative Screening helps bridge this gap. It offers ways to help you show respect, build trust, and connect with patients during health screenings.

We created the Conversation Map  to offer step-by-step guidance for a reliable, evidence-informed approach to screening about social needs and any other sensitive topics.

The Conversation Map integrates concepts and skills from motivational interviewing, trauma-informed care, and cultural humility to help you create safe, respectful screening conversations. This tool also includes sample language for each step of the Conversation Map.
