Motivational Interviewing for Social Care Navigators
Virginia Garcia Memorial Health Center is a federally qualified health center serving more than 47,000 people with a team of more than 600 employees. Virginia Garcia maintains a large team of social care navigators, who support patients with complex health and social circumstances. Ariel collaborated with the Training and Development team to develop an MI learning plan for the navigators, including both in-person and virtual support.
Person-Centered Intake in Oregon Community Behavioral Health
The process for getting people started in community behavioral health services has many rules and requirements. This innovation pilot generated significant momentum for changing the state administrative rules to create a more flexible, person-centered approach to intake in community behavioral health settings.
Online Learning for Health-Related Social Needs Screening
Kaiser Permanente care teams and staff across the country are learning to screen and respond to nonmedical social needs. KP Learn is the enterprise-wide Learning Management System, providing employees, physicians, and contingent workers with online access to learning resources. Ariel partnered with the KP Learn instructional design team to help develop an online learning module on health-related social needs screening.
Health-Related Social Needs Qualitative Evaluation
The Learning Health System program, led by the Center for Accelerating Care Transformation at the Kaiser Permanente Washington Health Research Institute, worked with two primary care clinics to launch universal social needs screening and conduct a mixed-methods, pragmatic evaluation assessing the comparative effectiveness of two social health support programs.
Motivational Interviewing Learning Series
Mosaic Community Health is a federally qualified health center in central Oregon that serves almost 30,000 patients a year with a staff of 400 people. Ariel partnered with the Training and Development team to develop a motivational interviewing training plan for a multidisciplinary audience. We offered both beginner and intermediate training so that team members could get an introduction to motivational interviewing, and deepen their skills if they were interested.
Health-Related Social Needs Practice Facilitation
The Learning Health System program, led by the Center for Accelerating Care Transformation at the Kaiser Permanente Washington Health Research Institute, partnered with care delivery leaders to facilitate the launch of universal social needs screening in two primary care clinics in Olympia and Spokane, Washington.
Motivational Interviewing Videos for PCA Learning Management System
The Community Health Care Association of New York State (CHCANYS) is New York's Primary Care Association, a membership organization representing New York’s 70-plus community health centers. Ariel created a series of motivational interviewing training videos for CHCANYS to distribute through their Learning Management System for community health workers at New York health centers to watch and learn about MI at their own convenience.
Primary Care Association Leadership Council
Craig Hostetler is the former director of the Oregon Primary Care Association. When he was called on to help primary care association (PCA) directors from around the country consider redesigning their approach to collaboration, he invited Ariel to help. After years of talking about change, PCA leaders were looking for some help to articulate a clear plan of action and get unstuck. After extensive listening and engagement with key stakeholders, we helped this group of leaders take decisive action.
Research on Health Equity Communication in Conservative States
The Delta Center for a Thriving Safety Net was a Robert Wood Johnson Foundation program to support state primary care and behavioral health associations. The Center for Accelerating Care Transformation (ACT Center) participated in the Delta Center as a technical assistance and evaluation partner and wanted to learn more about how state primary care associations (PCAs) and behavioral health state associations (BHSAs) are advancing health equity policies in conservative states.
Population Health Management Implementation Guides
Community health centers, Regional Associations of California, California Primary Care Association, Department of Health Care Services, and Kaiser Permanente are partnering to transform care for Medi-Cal beneficiaries through the Population Health Management Initiative. Ariel served as a partner to the series editor for this set of implementation guides to support health centers on Building the Foundations for population health management, and worked with a multidisciplinary team to produce these guides.
Online Learning for Person-Centered Health-Related Social Needs Screening
The Texas Association of Community Health Centers wanted to provide their members with a robust set of person-centered screening implementation tools through their Learning Management System. Ariel partnered with PCA staff to hear about their goals and their stakeholder priorities. We developed a suite of implementation tools for Texas Community Health Centers to promote person-centered approach to health-related social needs screening.
Hybrid Workshop on Person-Centered Health-Related Social Needs Screening
In their work reducing infant mortality and promoting quality improvement for Medicare beneficiaries, the Indiana Hospital Association supports their critical access, rural hospitals, and other rural health care providers with a variety of quality improvement strategies. Ariel has provided a series of trainings on person-centered screening to improve workforce readiness and confidence to facilitate respectful conversations about health-related social needs.
