Online Learning for Person-Centered Health-Related Social Needs Screening

COLLABORATIVE SCREENING

The Need As in many states, Texas community health centers are facing new requirements for health-related social needs screening.  The Texas Association of Community Health Centers wanted to provide their members with a robust set of person-centered screening implementation tools through their Learning Management System.  Our goal was to offer teams an easy, flexible format for getting prepared to ask these sensitive questions in a trauma-informed and respectful style. 

 

The Approach | Ariel partnered with PCA staff to hear about their goals and their stakeholder priorities.  We developed a suite of implementation tools for Texas Community Health Centers to promote person-centered approach to health-related social needs screening. We created training videos, system design and conversation tools, plus a User Guide for the tools for them to share online for health centers to access at their convenience.


 The Outcomes | One of their priorities for this project was to create a Patient Action Plan for navigators to use with patients.  To meet this priority, Ariel and Genevieve, our plain language expert, worked together to develop a user-friendly, interactive Patient Action Plan tool that incorporates health literacy and motivational interviewing to support dialogue, care planning,  and clarity on next steps. The Learnings | We loved working with an amazing team of communicators to assemble this suite of tools for Texas community health centers. We learned a little about the specific strategies they are using to address health-related social needs in Texas, and also learned that this work is very similar all across the country!
