HEALTH COMMUNICATIONS
The Need | Community health centers, Regional Associations of California , California Primary Care Association, Department of Health Care Services, and Kaiser Permanente are partnering to transform care for Medi-Cal beneficiaries through the Population Health Management Initiative. Together, these organizations are working to advance population health management capabilities in order to eliminate health disparities and improve the health of people and communities.
The Approach | We created a series of web-based implementation guides to support California community health centers across four domains:
● Empanelment
● Data Quality and Reporting
● Care Teams and Workforce
● Business Case
Ariel served as a partner to the series editor, and worked with a multidisciplinary team to produce these guides. Ariel had a particular focus on thinking and writing about how these domains for population health strategy are shaped by:
● New technology
● Care integration
● Focus on equity
● Payment reform

The Learnings | Ariel loved going back to her Patient-Centered Medical Home implementation roots with this project, and updating care transformation ideas from 10+ years ago. For example, even though a lot of the core ideas and concepts about empanelment remain the same, it was interesting to think about how telehealth, insights about social complexity, and extended care teams impact how we manage supply and demand in primary care.
